A 54-year-old woman presented at the hospital with headache and posterior neck pain, which worsened when standing or in sitting position improved supine position. diagnosis of rheumatoid arthritis was made age ofin 33 years, patient has been taking methotrexate methylprednisolone. Cervical MRI magnetic resonance myelography showed appearance CSF leakage, resulting a spontaneous intracranial hypotension. atlantoaxial subluxation also based on abnormal anterior atlas (C1) cervical X-ray image. The leakage corresponded region, indicated that hypotension caused by compression dura mater. These symptoms were following treatment intravenous drip extracellular fluids, she discharged from day 25. disruption matter induced is rare complication but worth considering determining etiology
